We have an Excel template to assist you in submitting your census information to us. When saving this file, please change the word “Template” in the file name to the name of your Company. The left column is used in conjunction with the error indicator table on the upper-left to explain what part of the line has an error. They should all be self-explanatory. Hover your mouse over the column headings for an explanation of the requested information. You may cut and paste from an existing spreadsheet to enter your data.

 

When you have finished, please confirm that there are no errors and e-mail the newly named file to compliance@rpgny.com. You may also upload the file via our secure file upload system by clicking HERE.

 

Please click below to download the template in Excel format and feel free to contact us if you have any questions. If you have trouble downloading the file, please e-mail us and we can send it via e-mail as an attachment.

 

NOTE: Please remember to add back in all pre-tax payroll deferrals when reporting Gross Wages. Also, you do not need to erase the first 'sample' participant.

 

The 2009 template now includes Ownership, Relationship and Officer columns.
